Job Description:

SOCIAL WORK - PART TIME -3 DAYS A WEEKResponsible for delivery of social services to residents and their families. Responsible for consultation and direction of Social Service in the skilled nursing facility.Develops, organizes and implements programs, policies and procedures, conducts case work services and counseling, recommends functions necessary to ensure overall operational viability. Maintains performance improvement activities within the department and participates CQI activities.

Responsibilities:

• Interpret social,psychological, and emmotional needs of the resident/family to the medical staff, attending physician, and other resident care team members
• Participate in discharge planning and make such plans available to the Administrator as required
• Provide written and or oral reports of the Social Work programs as required
• Atten and participate in the QA Commitee functions as required

Requirements/Qualifications:

• Must have, as a minimum, three years experience in a Social Work capacity at a hospital, skilled nursing care facility, or other related meddical facility

• Education

Mid Level (3-6 years)

• Experience

Bachelors Degree
