    **Job Summary: **

    We seek a dedicated and skilled Wound Care Nurse to join our healthcare team. The ideal candidate will possess in-depth knowledge of wound management and will be responsible for assessing, planning, implementing, and evaluating patient care in wound healing. The Wound Care Nurse will collaborate with physicians, nursing staff, and other healthcare professionals to provide comprehensive wound care, ensuring optimal patient outcomes.

    **Key Responsibilities: **

    - **Patient Assessment: ** Conduct thorough assessments of patients’ wounds, including type, stage, severity, and underlying health conditions that may affect healing.

    - **Wound Care Management: ** Develop and implement individualized wound care plans based on evidence-based practices, ensuring proper wound cleaning, dressing, and treatment protocols are followed.

    - **Education: ** Educate patients, families, and caregivers on wound care techniques, management, and prevention strategies, promoting health literacy and self-care practices.

    - **Collaboration: ** Work closely with interdisciplinary teams, including physicians, physical therapists, and dietitians, to coordinate comprehensive care for patients with complex wound issues.

    - **Documentation: ** Maintain accurate and timely documentation of patient assessments, care plans, progress notes, and outcomes in compliance with facility policies and standards.

    - **Training and Mentorship: ** Provide training and support to nursing staff on wound care procedures, techniques, and new treatments to enhance overall wound care practices within the facility.

    - **Research and Evaluation: ** Stay updated with the latest advancements in wound care management, participate in quality improvement initiatives, and contribute to research projects within the organization.

    - **Patient Advocacy: ** Act as a patient advocate, ensuring that patients receive appropriate care and that their needs and preferences are respected throughout treatment.

    **Qualifications: **

    - **Education: **Certification in Wound Care (e.g., Wound Care Certified (WCC), Certified Wound Specialist (CWS)) preferred.

    - **Licensure: ** Current and valid nursing license LPN/RN in Ohio.

    **Skills: ** Excellent clinical assessment and problem-solving skills, strong communication and interpersonal abilities, proficiency in electronic health records (EHR), and a commitment to ongoing professional development.

    - **Other Requirements: ** Ability to work independently, manage time effectively, and transport and manipulate patients as necessary.

    - Ability to perform physical tasks, including lifting, moving, and positioning patients.
