SUMMARY
This position is responsible for serving as the single point of contact by leading Yamhill Community Care (YCCO) efforts to identify and support community-centered activities to increase utilization and integration of community-based and clinic-based Traditional Health Workers (THW). This role supports the integration of community health programs and THW providers into care teams and treatment plans by performing the following duties.

ESSENTIAL DUTIES
Leads the implementation of YCCO’s THW utilization and integration plan including planning and facilitating meetings, creating buy in, and implementing strategic vision for advancing THW workforce, tracking decisions, actions and identified strategies.
Collaborates with providers and community stakeholders to improve regional access and availability of community based THW and health education services and supports.
Acts a liaison between internal and external partners to ensure accountability and responsible for maintaining and monitoring technical project plans, reports, and other supporting documents.
JOB DUTIES
Program Support & Coordination
- Coordinates with the THW workforce, plan partners and provider network to develop, pilot, and evaluate models to improve member access to and system integration of THWs.
- Collaborates with community organizations to develop and support ongoing patient self-management programs and the dissemination of health education.
- Aids in support of THW payment models.
- Monitors and regularly communicates project status to ensure progress toward completion and within agreed upon timelines.
- Proactively identifies barriers and challenges and develops recommendations with management and staff to discuss and resolve matters critical to THW integration and utilization.
- Organizes, facilitates, and participates in meetings with partner organizations to identify tools and tracking of baseline and future measurement and utilization of THWs across plan partners.
- Collects and synthesizes data, tracks, and reports on THW related Oregon Health Authority (OHA) contractual requirements and supports monitoring and evaluation activities.
- Responsible for participating in improvement plans and projects related to assigned functional areas.
Technical Assistance, Training, & Education
- Contributes to the ongoing review and analysis of the provider network and systems and identifies opportunities for improvement.
- Acts as a technical information coordinator to further the understanding of THW scope of work, care model, and oversight responsibilities.
- Makes formal and informal presentations and reports providing updates on THW related efforts.
- Tracks and promotes funding models being employed to reimburse for THW services.
- Works with management, staff, and external partners to develop and maintain project plans that outline clear goals, activities, timelines, deliverables
- Trains and mentors Community Health Workers (CHWs), serving as an advanced resource.
- Establishes and maintains professional and collaborative relationships with providers (physical, mental, dental health), OHA, other CCO Liaisons, THW Commission, and plan partners.
- Supports workforce development by sharing training and certification processes including technical aid on how to enroll as a provider and on-boarding new providers.
- Helps members learn new skills and approaches.
- Develops, in partnership with YCCO staff THW, related informational material describing scope of service, benefits and availability for the provider network, members, community partners.
- Resolves member and partner concerns by instructing understanding on access to services (e.g. specialty care, dental, NEMT, behavioral health, and appeals).
Community Engagement
- Convenes cross system/sector partners inclusive of Yamhill Community Care staff, health system and community stakeholders to develop and coordinate a regional THW integration and utilization system.
- Assists in preparing, implementing, and marketing programs and initiatives to enhance member engagement.
- Through case referral, supports members in accessing THW services in both clinical and community-based settings.
- Assists members in navigating to and utilizing THW providers and services.
- Collaborates with plan members, the healthcare team and auxiliary providers identifying members’ needs and challenges.
- Develops and maintains trusted relationships with members by being patient, open-minded, non-judgmental, compassionate, flexible, responsible, supportive, and demonstrating follow-through.
ESSENTIAL DEPARTMENT & ORGANIZATIONAL FUNCTIONS
- Participates in the preparation and submission of regulatory and contract required deliverables.
- Works closely with other YCCO departments, including Health Plan Operations and Quality Assurance and Compliance, to assist with audits – including the External Quality Review (EQR) - as needed.

- Reports member complaints to the appropriate authority for investigation and follow-up, per protocol.

- Maintains professional boundaries and effective working relationships with YCCO and staff, members, providers, and partners.
- Participates in work-related continuing education when offered or directed.
- Follows departmental and organizational policies and procedures.
- Performs other duties and projects as assigned.

- Knowledge of Oregon Health Plan (OHP) benefit package and OHA rules and regulations.
- Ability to work in an environment with diverse individuals and groups.
SUPERVISORY RESPONSIBILITIES
This position has no supervisory responsibilities.

EDUCATION & EXPERIENCE
Required:
- __Bachelor’s degree in health administration, public health, or health related field.
OR:
- Minimum of three (3) years’ program coordination in health systems or social service sector
OR:
- Any combination of education and experience that would qualify candidate for the position
Preferred:
- Traditional Health Worker training
