    Job Description

    The Youth Health Coach will play a vital role in supporting the health and well-being of students by providing education, motivation, and resources to promote healthy lifestyle choices. This position focuses primarily on preventing and addressing obesity among school-age children through direct student engagement, school-wide initiatives, and collaboration with clinical providers, educators, families, and community partners.

    The Youth Health Coach will function as an active member of the healthcare team by contributing expertise in nutrition and wellness education, to ensure students receive the best possible support. This position will be instrumental in teaching healthy eating habits, promoting physical activity, and supporting students and patients with special nutritional needs while working with school and corporate wellness teams to provide balanced and complete meal offerings.

    The essential job duties include:

    • Establish rapport with students, families, school staff, adult patients, and community partners to create a supportive health-focused environment.
    • Communicate with students, parents, and patients in-person, over the phone, via email/text, and in group settings to provide guidance on nutrition and physical activity
    • Lead group and individual health education sessions focused on obesity prevention and healthy eating, using evidence-based strategies.
    • Monitor students' wellness progress, track key health indicators, and collaborate with school nurses or counselors to provide necessary support.
    • Reinforce healthy habits and behavior changes, helping students set achievable health and wellness goals.
    • Participate in school and community outreach efforts to engage students in physical activity and nutrition education.
    • Provide education and resources on managing weight and making nutritious food choices.
    • Develop and implement individual and group wellness plans tailored to students' needs, including obesity prevention.
    • Teach students and families about evidence-based health guidelines, including nutrition and fitness.
    • Organize and participate in school health fairs, wellness screenings, and awareness campaigns focused on obesity.
    • Monitor students’ progress and collaborate with school staff to evaluate the effectiveness of health and wellness interventions.
    • Work as part of an interdisciplinary school health team, including teachers, counselors, and healthcare professionals.
    • Travel between WCPA school sites and WCHI clinic locations as needed to provide wellness education and coaching.
    • Provide in-service training to food staff on topics related to Nutrition and Foodservice.
    • Adult patients may be assigned as needed, based on capacity.
    • Travel may be required to Delano, Lost Hills and mobile clinics.
  • Qualifications

    Qualifications

    • Bachelor’s degree in Nutrition, Dietetics or related field
    • 1+ years of experience in health coaching, youth wellness programs, school-based health initiatives, or community outreach.
    • Written and verbal fluency in Spanish required
    • Proficient in Microsoft Word, Excel, Power Point and Outlook
    • Experience creating and customizing therapeutic menus and diet plans.
